It is estimated that the overall anterior cruciate ligament (ACL) injury rate in the US is approximately 200,000 per annum. This means that 1 in 300,000 people in the US will suffer an ACL injury for the first time every year.
ACL injuries commonly occur between the ages of 14 and 29 years, occurring in non-athletes and athletes. Those engaged in football, soccer, basketball, and skiing activities are most most likely to ACL injury .
Female athletes are more more likely to suffer ACL injuries than men performing similar sporting activities and training. Research shows that women are 2 to 8 times more likely to sustain ACL injuries compared to men.
So why are women more susceptible to ACL injuires than their male counterparts? In many respects the jury is still out but several factors have been attributed to this increased susceptibility. Women have a narrower femoral notch, that is space at the bottom of the thigh bone where the ACL runs. This tightness of fit may result in increased friction between the femur and ACL as the knee moves.
Another possible cause is the greater Q angle in women – i.e. the angle between the quadriceps and the patella tendon. A larger angle is partly due to a woman’s wider pelvis. This increases the risk for greater stress and ACL injuries.
Other potential factors include increased joint laxity, lack of strength and impaired neuromuscular coordination. Put simply, women are more vulnerable in executing the movements that typically lead to ACL injuries: for example, jumping and landing.
Women who take part in activities where jumping and landing are involved should work on preventing knee injuries occurring.
To manage the risk associated with these characteristics and increase the strength of the knee, women should include agility drills and plyometrics in their training programs. Women should include knee strengthening exercises that focus on strengthening the knee and the hamstrings, such as leg presses, squats and lunges. Learning to use the hamstrings instead of the quadriceps through muscle control techniques is also important for women. This can be done with OKC (open-kinetic chain) exercises.
